To request a Certificate of Insurance from our insurance broker Lockton, please download this Request Form and follow the instructions. You must always attach a copy of the contract, agreement, or permit application when you submit your completed form. If your request involves replacing an existing Certificate of Insurance, please include a copy of the original if available. Contact insurance@seattlearch.org with any questions.

View Compliance Form external organizations must sign before providing services involving minors under their own safe environment program. Keep completed forms in permanent file at parish or school office. If requesting a waiver of insurance, provide a copy of completed form to the Insurance Department (insurance@seattlearch.org).

• School Year - This form gives employee permission to tutor onsite and must be signed by the principal.
• Summer - This form gives employee permission to tutor onsite and must be signed by the principal.
